It seems likely that it's an issue with the compilation of makeindx (I believe
it was compiled with TurboPascal back in the day). Anyway, the version of
makeindex that is included with MikTeX (for win32) also dates back to 1993,
but is up-to-date on tetex-distributions. I honestly don't remember if Win98
runs 32-bit programs, but the are some more recent compiles of makeindex in
the \tex-archive\systems\msdos\emtex directory on CTAN. One of the files,
makeindx.zip, contains two versions of the executable that you might want to
try out.
