it's not a common problem (at least, in my experience ... i don't
support many people writing books...).

however, your guess about the age of the program sounds plausible.  i've
got a un*x makeindex here, latest distribution compiled in february;
would you like to mail me your .idx files and i can run 'em through this
recent version to see if it works.  if it does, we can investigate
getting you a recent miktex (or -alike) makeindex to run.
