I'm not an expert on hyperref - but have heard complaints before
about things showing up on the wrong page...    I belive Sue
had a problem with it as well...

how many times are you running latex?  It may be that you
should remove the "temporary" files and run the entire
thing some 3-4 times and then check it.  Things tend to
"move" while you are running it, and of course it reads the
last version of the  .<whatever> file it needs -

it would be an exercise - not too useful probably - to
copy your "temporary" files to   .aux.1, aux.2, aux.3
and see what the differences are...  and do the same with
the  .toc and index files...    I would have guessed you'd
have more trouble with a hyperindex than page refs...

look at your aux file and see what is in there...
