[XeTeX] XeLatex + Unicode + Footnote issue

DONE! Attached all the files.

It seems to work as you can see in pdf file.

However, MakePerPage (which is supposed to start the footnote number from 1
on every page doesn't work).

Appreciate if you can provide some insight into the overall problem.
