The following takes place on macOS Ventura 13.2 RC and a Mac with Apple Silicon M1 chip.
After activating weekly TeX Live update checks:
- Using TLU 1.54b3, there's a login item "Adam Maxwell" at the top of the list, with generic icon:

- Using TLU 1.54b6, this changes to login item "TeX Live Utility" in alphabetical order, with TLU icon:

Not sure what triggered this: launching TLU 1.54b6, or opening Configure > Schedule TeX Live Update Checks in it, or closing and reopening System Settings.
To be sure, I erased all TLU-related files I could find, namely the app itself plus
~/Library/Application Support/TeX Live Utility
~/Library/Caches/com.googlecode.mactlmgr.tlu
~/Library/HTTPStorages/com.googlecode.mactlmgr.tlu
~/Library/LaunchAgents/com.googlecode.mactlmgr.update_check.plist
~/Library/Preferences/com.googlecode.mactlmgr.tlu.plist
~/Library/Saved Application State/com.googlecode.mactlmgr.tlu.savedState
and restarted then reinstalled the app and activated the update checks.
Now, whatever I do (close and reopen TLU, change the update checks from weekly to daily, close and reopen System Settings, restart the Mac), it's always as in the second screenshot above: TeX Live Utility login item in alphabetical order with proper icon.
So things seem to be as they should, and my earlier observation the result of a mixture of old and new TLU setups.
