[OS X TeX] A real mistery

Attached, a pdf file (example.pdf) and a minimal tex file (sample.tex).

Open example.pdf with any pdf reader (Adobe Acrobat, Adobe Reader, PDF Expert) shows the label “A” in cmr10. 

Typeset sample.tex, and the label “A” disappears.

Wow.
