[OS X TeX] include graphics not working

Howdy,

Please create a minimal example file that displays the problem. Also, what preamble are you using? Also, what TeX Distribution are you using? Is it fully updated? Do you have old file in your personal tree (~/Library/texmf for MacTeX)? Supply your .log file.
