This problems also appends to me. It seems to be reproductible by
installing the standard fptex vesion (not the beta one) and then
upgrading just windvi with windvi-0.66-pre5.exe.
This problem actually disappears when installing mktexpk.exe and
gsftopk.exe of fptex.zip
