[dvipdfmx] missing annotation rectangle if the box contains no glyph

Perhaps LaTeX /cognoscenti /could be expected to known from that 
preamble that LaTeX or XeLaTeX is required, but the man on the top deck 
of the Clapham Common omnibus (of which I am a fairly typical example) 
would be hard pushed to tell, especially as the specials are of the form :

> \special {pdf:bann}

from which I, as a naive user, naturally assumed PdfTeX.
