[texhax] Index in Table of Contents

Dear Barbara,

It's nice of you to take all this time!

I've been using Tex and Latex since the mid-80s.  I've learned that Don
Knuth is not the only generous Tex user.  The entire Tex user community
seems to consist of people who share his devotion to producing beautiful
manuscripts and are willing to take great pains over others' manuscripts.
