Do you mean MacTeX-2008 or plain TeX Live?

 From your original console output:

/Users/hans/Library/texmf/tex/latex/graphics/graphicx.sty

so you DO have junk in your personal texmf tree (~/Library/texmf)!!!!  
Get rid of that stuff!!!

Sigh... I hope you've left off the /usr/ since they should be /usr/ 
texbin ad /usr/local/bin.
