Could please specify once clearly what is the problem you want
to solve? It is also a good idea to create a new subject header
stating this problem in brief.

Examples:

1. I want to create forms that can be filled on-line via
   HTML browsers. (Use LaTeX2HTML or TeX4ht).

2. I want to create forms that can be filled and printed
   using TeX. (For this Krishnan has provided a solution).
