I already tried and it is working. The problems are (1) the heading is to be changed to "List of Cases" instead of `Index', (2) The list is coming in two columns which should be in a single column.

I will try it. But the problem may be for 'lot' it looks for \caption in the table environment. But I have not created any case environment.
