[texhax] LaTeX package for boxes/forms?

Hi all,

I am trying to recreate my own version of
http://dediserver.eu/downloads/example.pdf
for which I need two things:

1) A way to create nested boxes which align nicely, preferably with
nice captions in
them.
2) A way to create the dotted lines, preferably with 2.5 mm distance
in the printed
version.

I do know LaTeX, but I never had to do this kind of formatting, so I
simply don't know
what to do. Looking around CTAN did not yield anything of value (to me, with my
limited knowledge of boxes etc in LaTeX), either :/
