[texhax] appendices page numbers in TOC and problems with hyperref+pdfpages

This is a well known problem.  Don't expert a solution in the near
future.  It's not trivial.  There are some nasty things in the Adobe
PDF specifications which make life much harder then necessary.

I really hope this problem will solved in the near future but I think
that we have to be patient.  Developers are involved in much more
important things at the moment.
