    submitting a fix to a latex package, 

In general, the best thing is to write the maintainer (if known).

    as well as posting a modified package to CTAN.

If the package is still actively maintained, then it's usually best to
let the maintainer do the update, to avoid multiple versions.  If there
is no maintainer, then the best thing to do depends on the licensing,
etc.  What is the package?

In general, uploads to CTAN can be done through a web form, for example,
http://www.ctan.org/upload.html, among other ways.
