I suggested in a past message that transparent overlays would give the effect of
returning to the top of the page, but they do depend on setting the content of the
overlay in advance. If the content of the overlay is generated as the bage is built,
the solution is probably in a two pass system, where the content of the overlay is
a dummy the first time through, and is then picked up from an auxiliary file the
next time through.
I sometimes have as many as 5 transparent overlays on an article first page, so that
headers, section numbers, acknowledgements, etc., are in fixed position even when the
author makes a major change in the first regular text paragraph.
