[tex4ht] what would cause " Can't close file `index.tmp' (file is not open)" --- error --- Illegal storage address

Hello Karl;

Thanks for finding the problem!

I am OK now. I finally upgraded to TL 2018 and have not
seen this problem again.  So I am all set now.  No more

        Can't close file `index.tmp'

It seems to only happen on TL 2017 and under special cases.
