The error on the first run was probably due to a sync operation running
between the mirrors at the same time. During this time, inconsistencies
might happen, because the CTAN mirror network uses rsync to bring files
uptodate, but this is not an atomic operation.

So yes, it can happen, but should disappear normally rather quickly.
