[tex-hyphen] hyphenation patterns with apostrophe

Hello,

It would be nice and it would be time to solve the ignored issue with
apostrophe. At the moment XeTeX uses U+0027 only and LuaTeX uses
U+2019 only.

I'm thinking about duplicating all the patterns in hyph-<lang>.pat.txt
to include both U+0027 and U+2019 (that step would be done during
conversion from hyph-<lang>.tex to hyph-<lang>.pat.txt). This doesn't
address the issue in XeTeX yet, but it would solve it for LuaTeX.

For ConTeXt convertet this implies that:
- lua files for MKIV could keep all patterns as they are
- tex files for MKII could strip/ignore/remove all the patterns with
U+2019 during "conversion"

Any thoughts about that? The issue with XeTeX has to be solved on a
different level, but we can only deal with a single item at a time.
