Buddy, I don't see any problem in your log, except for the final error. I tried running the document on a MikTeX 2.7 system, without problems. Perhaps the file was damaged by the mailer, or you were hit by line ending issues. I posted a version as http://math.ucsd.edu/~msharpe/numberformat.zip in case you want to try with a clean file.
