Found it!
On the iBook I get:

And on the x86 FC4 system (which is probably using A4 paper, not US Letter):

Note that the iBook greyness, thickness, and (x,y) changebar coordinates are
all 10x higher than they should be. I manually set them to 10x smaller and
the PDF output was correct.
Does this mean that the iBook build has (for some reason) got the wrong
value for fixed_point_decimal? I'll take a closer look at the build system.
