Just using an old version of TeX. Your example works using tetex-2.99.4
(beta test for tetex-3) on SGI mips-irix with the TL2004 texmf trees,
which gives a newer pdftex:

pdfeTeX, Version 3.141592-1.20b-rc3-2.2 (Web2C 7.5.3)

but the same hyperref:
Package: hyperref 2003/11/30 v6.74m Hypertext links for LaTeX
