[pdftex] pdf form example

[MIGAR5]

Dear colleagues,
I'm trying to make a form, with some input fields and a 'submit' button
using the hyperref package for pdfLaTeX.

but i get an error like this:

! Undefined control sequence.
\PDFForm at Submit ...F << /FS /URL /F (\Form at action
                                                  ) >> 
\ifForm at html /Flags 4...

Any Ideas? Could you tell me where i can find an form example?
