For the record, this problem went away when I updated my teTeX.

I use the TeXShop software for Mac OS X and with a distribution of
teTeX put togehter by Gerben Wierda. see
<http://www.uoregon.edu/~koch/texshop/texshop.html>. Once I updated
my installation of teTeX the problem went away. The problem was,
apparently, not in my set-up, but it was something that either Gerben
Wierda fixed in his modifications to teTeX or someone working on
pdfTeX fixed between "This is pdfTeX, Version
3.14159-14h-released-20010417 (Web2C 7.3.3.1)" and "This is pdfTeX,
Version 3.14159-1.00a-pretest-20010806 (Web2C 7.3.3.1)"

Thanks so much to C. V. Radhakrishnan for his help.
