[pdftex] Problems with "index" + "hyperref"

I need "index" package to define an author index.
When I run "pdflatex" or "latex" on a latex file using "index" package together with "hyperref" package,
I always get this error message. Are sombody using "index" and "hyperref" packages together successfully?

I tried with "testslide.tex" which comes with "hyperref" package in "test" directory.
I just added "\usepackage{index}" and commented out "\usepackage{makeidx}",
since these two conflict  with each other. Then I have the above mentioned error.
I am using the latest "index" and "hyperref" packages.
