After some poking, it appears to me that the “v” to the right of the title is just gives you a minimalist file browser window. Any time you want to, e.g., open any file, CMD-O (or File -> Open) pops up a browser window with a default directory, which is likely the last directory you referenced in any such operation (open, close, save…). I think this is borne out by messages earlier in this thread.
Turns out that another thing I mentioned, that the doc icon to the left of the title was insert, is just because the original of the document (a JPG) came from a mail enclosure, and was long gone from the file system.
So my 10.13 comments were bogus.
