Typeset sample.tex, and the label “A” disappears.

The label A is not actually in the file; at least not anywhere that I could find,
using Acrobat Pro and Illustrator CC.

Try Zooming-in on the A (using Preview, say).
The A disappears.

Although there is a font subset of Myriad-Pro in the PDF, the A is not using this font.
(see image below)

So I think that there must have been a preview added to the file,
which is used rather than a complete re-rendering, when the image
is viewed within a certain size range.

If example.pdf is opened in Illustrator, there are no text objects.
Save under a new name (say, example-2.pdf) and the letter A has gone.
