I'd first try the command
sudo -H texconfig-sys help
It will provide some hints. Or, for personal purposes:
texconfig help

What is missing? Any additions should be put into the /usr/local/
texliv/texmf-local branch and made available via texhash etc.

Yes. But then you'll have a TeX (Live) system that is almost one year
old. MikTeX Package Manager, mpm, can update some packages.

No, you can correct texmf.cnf. And it's the default when you install
from CD or DVD.
