It's partially outdated, but the adaptation to present-day TeXLive  
shouldn't be too difficult. Regarding the conversion to .pfb and .afm,  
see especially

That said, with the advent of XeTeX (and Textures soon) there is  
really little incentive to go along this route any longer, apart from:

- being a purist,

- being a masochist,

- needing LaTeX packages for which no XeTeX supports exists yet,

- needing sophisticated math font trickery for which XeTeX is not yet  
well suited.
