Howdy,

Generally. But not usually in the root of that directory.

Not quite. /usr/local/gwTeX/texmf.pkgs/tex/latex/

Yes.

By the system when setting things up. E.g., when formats are  
generated by the system, for the map files generated by updmap-sys,  
etc. Not for local additions.
