[OS X TeX] Making forms

I'd appreciate suggestions on what to use to produce forms (tick- 
boxes etc.) with LaTeX.
Preferably, I'd like to be able to both print a nice looking form and  
have a real pdf form that could be used electronically.

Any suggestions are welcome.
