My misunderstanding. The --force flag is required:
ruby
/usr/local/teTeX/share/texmf.tetex/scripts/context/ruby/textools.rb
--fixtexmftrees --force ~/Library/texmf
To force a fix of the texmf.local tree:
sudo ruby
/usr/local/teTeX/share/texmf.tetex/scripts/context/ruby/textools.rb
--fixtexmftrees --force /usr/local/teTeX/share/texmf.local
