[l2h] 'About this document..." appears on contents all content pages e xcept the first

I have 5 subject html pages under contents.  as i page thru each of the 5
html pages the bullet "About this document..."  appears on all the pages but
the first.  Does anyone know how to eliminate this bullet (or what causes it
to be added to the pages)?
