[l2h] Bug in first hyperlink of TOC

Hi Ross,

thanks for your answers and for your work on LaTeX2HTML.

>From documentation I was not aware of the manifold
configuration possibilities; especially I didn't find
anything in the context of style-sheet, configuration file
or even "hook" (if I had suspected such a feature) in the
docs.

Maybe one could add additional subsections about
"Configuration file" and "Style sheets" below section 5,
which could discuss such topics as from my mails.

Another section would also be nice:
"Generating standalone HTML documents"  =8-)
