Both problem seem to have the same source - the value of $PSTOIMG seems to
be undefined. Please check your latex2html.config for this and the correct
value of $LATEX2HTMLDIR in the latex2html main script.
By the way: There is 99.2beta available at
http://saftsack.fs.uni-bayreuth.de/~latex2ht
You might try that one, its configuration should be much more robust.

Hmm, exactly which prompt do you mean? And with what options do you run
latex2html? One could think of an additional -force switch...
