You have to update pdftex.def, not the graphics package:
  CTAN:macros/pdftex/graphics/pdftex.def
  http://www.tug.org/applications/pdftex/pdftex.def

You are mixing files of different hyperref versions.
Ensure, that all files of hyperref, which are loaded, belong
to the same version.
