[Fontinst] Fontinst v1.928 released

I've uploaded a new version of fontinst to CTAN (minimally incrementing the
version number to 1.928).

The changes are rather minor, and can be summarised as follows:

1. The manual has been extended.

Several of the commands which were previously only listed now have proper
descriptions, although there are still some left to describe.

2. A couple of bugs, mostly in fontdoc.sty, have been fixed.

3. A new conversion command \etxtocmap has been added (to finstmsc.sty).

CMaps can be used in PDF files to assign semantic meaning (Unicode
characters) to the slots of a font, for searching and copying text.
\etxtocmap can generate a .cmap file from an ETX file, where it takes the
semantic information from \Unicode slot commands.
