I've recently solved a similar issue for one of TeX4ht users. He used the
Booktabs package, which provides \toprule and \midrule commands, which can
be used to determine whether we are in the table header. I haven't added it
to TeX4ht sources yet, but I can if there is a demand for it.

See the attached .tex and .cfg files. You can compile them using:

$ make4ht -c config.cfg sample.tex
