I do indexes all the time using pdftex, makeindex and eplain.tex. 
That is my normal mode of indexing a customer's pdf file. I save 
the pdf file as text, add \input eplain.tex,  change all the EOP 
markers to \vfil\eject, set the starting page number and then index 
away. I can't do them with eplain in Context because of a conflict. 
But Context has its own built in indexing capability. 

The eplain method allows you to print a pdf copy of the file with 
marginal indications of the index items on each page. Suggest you 
look at the eplain documentation, page 18ff.
