[texhax] Cannot use \makeindex and pst-pdf

You need to hide things that don't work in the latex portion of the
processing:
\usepackage{ifpdf}
\ifpdf
\usepackage{index}
\else % provide dummy replacements
\let\index=\relax
\let\printindex=\relax
\let\makeindex=\relax
\fi
