The same I got last night with MiKTeX 2.4. It worked when I kept to
the usage note above: "weave inputfile changefile outputfile"

  weave tex.web tex.web tex.tex
  pdftex tex.tex

No idea whether the second "tex.web" is correct, but this finally
produced a pdf.
