I have never done it and I hope never to have to, but there is a quite 
interesting and detailed article in the TugBoat I got last week (I think 
it was actually dated 2003, but it just came out) on the subject.  The 
author was dealing with a publisher who would take nothing but Word and 
who owned the copyright for the original edition, so changing publishers 
was not an option.  (So there are perfectly valid reasons for wanting to 
do this.)

Basically, he used something called VTeX (a commercial program) to go from 
LaTeX source to HTML and then used Word to import the latter.  He did not 
that he had no mathematics in his 755 page ms, however.  

I think if I were faced with this problem I would give the publisher a 
printout and invite him to typeset it.
