Interesting. We just did a book which had inset topic arrows somewhat 
like to that, but the publisher was quite fussy / explicit about 
where they wanted them, so we didn't bother to work up a general 
solution.

It's kind of hit-or-miss as to which articles are up for the older 
ones, and I've encountered a couple where they .pdf wasn't useful 
directly 'cause fonts weren't embedded---looking into fixing that is 
a project on my back burner.
