To be fully registered, you need to either print a copy of this electronic form after filling it out; or download and print the form in portable document format (pdf). Mail or fax the completed form to the TeX Southeast registration address by July 15, 2002. Please be sure to include payment by check, or your credit card information and signature. See the field descriptions below for valuable information.

Accommodations (please read carefully)

If you require accommodations for the conference only, please fill out the information regarding your check-in and check-out dates. Please remit the conference fee plus the fees for the accommodations when you register.

If you are attending the LaTeX class but are local and do not need accommodations, be sure to check the "No Accommodations" box next to class enrollment; this will lower the fee.

If you are attending both the LaTeX class and the conference (and are not local), only fill in the dates needed for the conference. Remember that the class fee includes the accommodations, but only for the nights of the class.

FIELD DESCRIPTIONS
Conference
The conference fee includes breakfast, lunch, breaks, and preprints; room cost is extra. Guest accommodations will be located in Building 3A (The Language Dorm). If you are attending the conference and have special food needs (e.g. kosher, vegetarian, etc.) or any questions in general, please contact us at tex-se@tug.org.
LaTeX Class
The class fee includes breakfast, lunch, dorm room, and linens. The nights covered are July 28th through August 1st. If attending the conference as well, you must schedule lodging for August 2nd and beyond as desired.
If you do not require room accomodations, be sure to check the "No Accomodations" checkbox to save the room costs.
The class will cover basic LaTeX through intermediate and just touching on advanced. If there are specific question or problems students are having, they may bring examples with them to work on in class.

Other Lodging
For those of you who do not wish to stay in the dorms, you may search lists of local hotels in your price range.
Travel Information
Raleigh/Durham Airport is the closest airport to Duke University. Look at http://www.raleigh-rdu.com/ for airport information such as ground transportation, rental car information, etc.
Bursary
The bursary contribution provides financial assistance to those in need who wish to attend the TeX Southeast conference.
Information about you

Payment
For credit cards, only VISA, MasterCard, and American Express are accepted. Your credit card information is collected and stored securely. If you wish to pay by any other method, or wish to pay by credit card via fax or traditional mail, please print the PDF application form and follow the instructions on that form.
If you wish to electronically submit credit card orders, you do not need to fax/mail this form in addition to the submittal.
