I think you do not have to run bibtex at all, as everything 
is in your document.
bibtex is useful when you collect many reusable references 
in a bibliographic archive.

Just run 2-3 times pdflatex, for cross referencing, and it 
will do.

The same for makeindex as Herbert said.
