via this online form, please fill out this form, then print a hard copy of the filled out form
from the browser window, and fax or mail to the TUG office (address and fax number below).
Notes:
- Information about you. Enter all
items carefully: this information will be used for mailing
publications and other notices to you. You should receive a
confirmation of your application at the email address you provide.
- Submit. After you've entered all the
information, click the Submit button above to securely send us
your application and payment information.
- Secure Server.
You can assure yourself that your sensitive personal information will
not be acccessible to anyone but TUG: examine the icon in your browser
window's lower left corner: if it is a "closed padlock" (under Netscape
Communicator) it indicates that your browser will use the Secure Socket
Layer to communicate to our server. Always insist on adequate security
before clicking the Submit button on this or any web form.
- Payment.
For credit cards, only VISA, MasterCard, and American Express are accepted.
Your credit card information is collected and stored securely.
If you do pay by credit card, you do not need to print/mail the
form; your electronic submission suffices.
