You can find the details on p.24, section 3.4.1, in the smaller of the
two manuals: `Making new TeX formats -- running ini-TeX.

If these aren't clear enough, I have some really low-level ;-)
plodding instructions on doing an initex run, which I actually used
just a few weeks ago ;-) to set up French hyphenation. Oh, that's what
my instructions are for -- an initex run for adding hyphenation
capabilities. This may be too specialised for your purposes.

In any event, start with section 3.4.1 and see if that rings any bells
for you ;-)
