[XeTeX] Depth of an index entry.

How can I have an index entry to a depth of 4? This works -
╭────
│\index{ginseng!side-effects!nausea}
╰────
but this doesn't -
╭────
│\index{herbals!ginseng!s/e!insomnia}
╰────
I am using -

\documentclass{scrreprt}
\usepackage{imakeidx}
\makeindex[options=-s ./index.ist,columnseprule,intoc]
\usepackage[colorlinks=true,linkcolor=blue,citecolor=green,linktoc=all,hyperindex=true]{hyperref}
--8<---------------cut here---------------end--------------->8---
I've been looking at "imakeidx.pdf" but can't see anywhere where the
depth is defined, so I'm stumped.
