Your mini-sample-e.tex compiles with no problem for me in cygwin-1.7. 
I've attached my log so you can compare it with yours.

I think it's clear at this point that there is something wrong with your 
TL2009 installation, probably involving more than just map files.  I 
would suggest that you ask on the texlive list for advice on the best 
way to recover.
